Forms
This section contains all the forms that are required to be completed by different parties at various stages of the dispute resolution.
Resolution Facilitation Request: Form: It must be completed and signed by all parties to a dispute parties wishing to initiate a resolution facilitation process.
Request Form: It needs to be completed by the claimant in order to initiate any proceedings.
Answer to a Request Form: It needs to be completed by all parties to a dispute, in response to the Request Form filed by the claimant;
Intervention Form: It needs to be completed by anyone who has not been identified as a party to a dispute to request participation in the process.
Application for Provisional and Conservatory Measures Form: It must be completed when a party to a dispute wants some measures to be taken to prevent the occurrence of irreversible consequences while waiting for the decision to be rendered.
